The Commonwealth Healthcare Corporation (CHCC), with guidance from the U.S. Centers for Disease Control and Prevention and the U.S. Food and Drug Administration, will be offering 12 to 15-year-olds the Pfizer-BioNTech COVID-19 vaccine at the MCAT vaccination site starting May 14, 2021.

Walk-ins are accepted at the MCATS and parent consent forms are required for minors who are unaccompanied by a parent or legal guardian. Parent consent forms are available at www.vaccinatecnmi.com. It is requested that a valid government issued ID or birth certificate be presented at the time of registration.

Transportation to and from MCATS is free and available for minors with signed parent consent forms. To avail of the free transportation, register online at www.vaccinatecnmi.com, or call 682-SHOT (7468).

12 to 17-year-olds who complete their 2-dose series will receive gift bags.
The operation at MCATS will run Monday to Friday, 8 a.m. to 6 p.m.; and Saturdays, 8 a.m. to noon. School-based vaccination is being planned at selected schools, to be announced at a later date.

Additionally, a comprehensive vaccination service is available for 12 to 17-year-olds. All individuals who avail of this service will receive the Pfizer-BioNTech COVID-19 vaccination, plus all pending routine vaccination shots.
